The Milwaukee City Conference has upgraded its online registration process!

Beginning with this 2026-27 school year, all student-athlete registrations must go through the school-affiliated online registration portal. Please find your school below.

Please note: even if your school co-ops with another school to compete in a sport, please complete the student-athlete registration through your traditional day school.

What to Know to Register for High School Athletics

The Milwaukee City Conference online registration process requires three steps for all high school student-athletes.

All student-athletes must have or obtain a physical examination. Physicals must be completed within the past two years to meet athletic participation requirements. Student-athletes may not participate in any team activities until the school’s athletic director has received and verified an up-to-date physical.

How to get a physical

  • An athletic physical examination can be completed by your child’s pediatrician or primary care provider.
  • Concentra Clinics provide free physicals for all Milwaukee Public Schools student-athletes.
  • A nurse practitioner (NP) who is qualified to perform athletic physicals

Before scheduling, confirm that the provider offers sports/athletic physicals and can complete the required paperwork.

All student-athletes must have a valid insurance card on file. Students who are uninsured may apply for coverage at K&K Insurance for an affordable rate.

All student-athletes must register online through their school’s online registration portal (see the links above). Before starting the online registration process, please make sure you have all required personal and medical information on hand. This includes:

  • Full legal name
  • Date of birth
  • Emergency contact information
  • Student’s physician’s name and phone number
  • Known allergies
  • Past medical history

During the registration process, you must submit clear pictures of both the front and back of your insurance card.
